Your personal information in this book may help doctors,nurses and paramedics save your life!
The book has details about medications you take, your doctor's contact details and any medical conditions you have.
Comes in a plastic sleeve to protect the book in your bag and with magnets to keep it on your fridge door where the paramedics can easily find it.
This project has been completed with the distribution of booklets to people living in retirement facilities.
